HealthSciences Institute's Chronic Care Professional (CCP) Certification Program

Better health care depends on the delivery of
evidence-based clinical services and evidence-based health improvement, disease management, and health coaching strategies and interventions. Yet, most disease managers, case managers and health care providers bring limited training and experience in these key practice areas.

In 2004, HealthSciences Institute introduced the first evidence and competency-based professional disease management and health coaching curriculum in the US--designed in collaboration with a expert interdisciplinary national advisory board and association partners. The Fourth Edition of the program was released in 2008.

CCP Certification program is the only nationally recognized population health improvement, disease management and health coaching certification and has been selected by DMAA: The Care Continuum Alliance as the recommended credential for population-based health and disease managers.

1. What is the Value of CCP Professional Certification?

Population health improvement, disease management (DM) and health coaching are rapidly growing fields in health care. Yet, few professionals bring training or experience in DM foundations, best practices, and evidence-based approaches and interventions.

The CCP program includes a core curriculum designed to prepare case managers, disease managers and health providers to deliver improved quality and value to patients and purchasers. The program addresses not only chronic diseases, but key considerations for supporting senior health and managing age-related conditions.

CCP certification demonstrates that professional staff have mastered a core DM curriculum (defined by a third-party certification organization, designed in accordance with national standards, and based on a national competency modeling study) -- and successfully completed a certification examination.

The CCP program has been recognized by DMAA: The Care Continuum Alliance and the Case Management Society of America as the recommended credential for health and disease managers.

Organizations who require their professional staff to complete CCP certification and who are approved organization partners of HealthSciences Institute, may request a jpg file of the CCP certification seal for use on organization web sites, promotional materials, or proposals.

  • Value for DM & CM Organizations

The number of disease management (DM) and case management (CM) organizations and providers are growing. If you're a DM or CM organization, you can distinguish yourself from the many vendors in this field by documenting that your professional staff have completed a core DM curriculum and have been tested on their knowledge by a third-party certification organization.

Using the CCP program as a foundation for your organization's internal training can also help you reduce employee development and onboarding costs. Further, the eLearning program provides a virtual classroom for staff at work or home, 24/7 -- minimizing costs of travel, conferences, and time away from work.

  • Value for Health Plans or Employer Purchasers

To improve HEDIS and public health care quality reporting outcomes (and costs), it's critical that your staff be prepared to deliver the latest evidence-based disease management services to consumers. CCP certification ensures that your professional staff can fulfill your organization's mission to improve quality and manage costs.

  • Value for Health Care Provider Organizations

If you're a provider organization, certification can help you succeed in the emerging performance-based health care environment. Payers are looking for providers and provider organizations who can deliver measurable value to consumers. Host the live chronic care professional program to build your high-performance health team.

  • Value for LTC & Home Care Organizations

The long-term care (LTC) system is changing rapidly. LTC and home care providers who can support senior health and independence -- across acute and long-term care settings will be best positioned for these changes.

  • Value for Individual Health Care Professionals

Population health improvement, disease management and health coaching are rapidly growing areas in health care. If you're a professional, certification can help you demonstrate your knowledge in this area -- and can help prepare you for the many new employment and practice opportunities in this growing field.

2. Who Should Register for This Program?

Because health and disease management are by definition interdisciplinary and consumer-focused approaches, the CCP program is designed for a range of disease management, provider, and health coaching professionals, e.g., nurses, physicians, pharmacists, rehabilitation therapists, respiratory therapists, psychologists, social workers, exercise physiologists, dieticians, working in health plan, employer-based purchaser, public health, primary care, hospital, disease management, and long-term care organizations and settings. As an interdisciplinary program, there is no prerequisite professional or licensure requirement for CCP certification.

3. What is "CCP Certification?"

CCP certification is the only nationally recognized population health improvement, disease management and health coaching certification. It is designed to augment the specialized training and skills of health care providers. Unlike most certification programs the CCP program is consumer, not discipline focused (population health improvement, disease management and health coaching are by definition interdisciplinary). And unlike DM program certification offered by URAC, NCQA, or JCAHO certification, this program is geared toward professionals, not programs.

Individual professionals completing the program are invited to list "Chronic Care Professional Certification" when documenting professional training and certifications. Individuals completing CCP certification may also use "CCP" as a designated credential. (As always, professionals should refer to the federal, state, employer, or professional guidelines or statutes governing their profession or role to determine the specific chronic care services they may or may not provide).

Further, organizations whose professional staff have completed CCP certification may include the CCP seal on their organization web site and promotional materials. Following delivery of the program to your professionals, please contact us to request HSI and CCP logos in jpg format.

CCP certification is valid for three years. Re certification through re-examination or documentation of 15 hours of continuing education in chronic care-related areas may be obtained for an administrative fee of $75.

4. How is the CCP Program Delivered?

The CCP program can be delivered as a stand-alone offering or integrated with existing new hire orientation or employee development programs. Two delivery methods are available:

The live program covers core CCP program content with a strong emphasis on building patient partnering, behavior change, and health coaching competencies. The on-site program is available for groups of 25 or more. HealthSciences Institute also offers national Pre and Post Conference CCP Events . Although the live session program format may be customized to fit the needs of sponsoring organizations, a Sample Agenda is available for review.

The eLearning program is designed for organizations that wish to minimize travel expenses, offer just-in-time training, or reach dispersed or home-based employees. The eLearning program is also an excellent choice for individual registrants. Registrants have 12 months following enrollment to complete the program and exam. All learning is self-directed.

Why Should Your Organization Choose CCP Certification?

1. Build Critical DM and Health Coaching Competencies

The World Health Organization and the US Institute of Medicine have called for the revamping of professional health care training and continuing education programs to better prepare professionals to drive population health improvement, support disease self-care and promote behavior change. Yet, most health support and direct care professionals still bring little or no formal training or experience in population health improvement to their new roles. Even experienced professionals may not be applying the most strategic, effective and efficient patient partnering and care strategies.

2. Demonstrate That Your Professionals are Prepared to Deliver Value

Increasingly, organizations of all types are offering population health improvement, disease management and health coaching services. However, evidence suggests that program effectiveness requires prepared health care professionals who can apply evidence-based guidelines and work effectively and efficiently to engage patients and support healthy behaviors through proven, structured interventions. CCP certification distinguishes your organization from others who may be offering similar services.

3. Leverage Your Internal Training and Development Resources

The CCP program is a comprehensive, competency-based program that has been designed in partnership with leading instructional design, adult learning and eLearning professionals. The regularly updated program reflects the best evidence and best practices in the field. By offering CCP certification to your staff (combined with targeted in-house training on software, systems, programs, procedures and policies) your organization can focus on delivering improved value to patients and purchasers--rather than developing core employee competencies.

4. Select a Practical, Evidence-Based Program

As with medical care interventions, health support and health coaching interventions should be evidence-based and easily applied by health care professionals. While CCP certification is not the only health coaching certification program, it is the only evidence-based program designed by clinicians and experts who have implemented programs and delivered health support services to patients.

5. Choose an Acclaimed and Recognized Certification Program

More than 95 percent of professionals completing the CCP live and eLearning programs agree or strongly agree that the CCP program leaves them better prepared to deliver health improvement, disease management and health coaching services in their day-to-day job roles. And an equal number agree that the CCP program represents a “good value” for their organizations.

And, a 2007 DMAA and HealthSciences survey found that nearly one-third of surveyed US health plans and disease management organizations require or recommend CCP Certification for their professional staff.

6. Join Leading Health Care Organizations in Offering CCP Certification

The CCP program has been chosen by US and international organizations--and individuals from the leading health care organizations.
